Login to the Website
Go to Dashboard or Admin > Publishing > Folders
Select the article you would like to create a form or Add a new article
Edit the article
Select Create Form from the drop down menu
Click on the pencil icon to edit form
Complete all the details as desired
Insert "Heading Text:" (Optional)
Insert "General Text:" (Optional)
Check Box "Disable focus on first form field" (Optional)
Select Response Type*: (Mandatory)
Enter Pop up message if you have selected "Pop up message"
Select Redirect Article if you have selected "Redirect article"
Style of field labels
Complete these fields if forms on your website are not styled using CSS
Enter "Send submitted data to an email address" (Optional)
Enter "Email Subject:" (Optional)
Form Creator Fields
To Add fields
Click on "Add Field"
Complete the details of that field
Click on "Save Field"
To Save and add another field
Click on "Save & Add New"
Once you have completed the Form click Save and Check in or Publish (CM 5.7 versions and higher)
To Edit Fields
Click edit field
Once you have made the changes to the field click Save Field
Then click Save and Check in or Publish (CM 5.7 versions and higher)
To Delete Fields
Select the field you would like to delete
Click Delete field
Once you have deleted the fields click Save and Check in or Publish (CM 5.7 versions and higher)
